• Complete the NEW PROBLEM form on-line if possible OR
• Complete the Service Request / Problem Log paper form found in your main office.
  
• Do NOT make verbal requests or phone calls without completing one of the above forms first.
  
  Documentation of problems with our computer system is necessary to monitor trends throughout the organization and allows us to best utilize current staff and determine future staffing needs.
